REACH is a scattered-site “housing first” program with the objective of moving chronically homeless individuals directly from PSI emergency service systems into permanent housing. REACH Service Coordinators will work as part of an intensive case management team serving program participants who may be experiencing serious medical problems, untreated mental illness, active drug and alcohol addiction, and other major health challenges. REACH Service Coordinators will use a harm-reduction approach to working with participants, including the utilization of Stages of Change Theory, and Motivational Interviewing techniques. The Case Manager for the Housing Supports Program will provide comprehensive case management and motivational interventions for chronically homeless individuals transitioning into permanent housing in scattered site and congregate settings according to the program’s established goals. The case manager takes referrals and conducts screening for placement in PSI’s Housing Supports Program project sponsored by Substance Abuse and Mental Health Services Administration. The case manager is the primary point of contact for each assigned client and is responsible for working with the client to develop an individual service plan designed to support the client’s long-term stability in housing. The case manager links project participants with mental health and substance abuse treatment; treatment for trauma and co-occurring disorders; and conducts follow-up activities. The case manager is responsible for coordinating housing referrals for eligible prospective participants and will partner with Outreach services to transition individuals into housing.
